脊椎 Part 17 骨盆构造

Sacroiliac joint是相对坚固却稳定的组织,可以让外力有效的从脊椎和下肢互相传导。 骨盆是类似一个环状,让脊椎和下肢的力互相传导。 Sacroiliac joint容易受到长短脚或不正确的坐姿而受到不对称的外力,最终造成腰椎疼痛等问题。 维持这个关节的稳定度除了因为关节的形状,里边的一些关节囊,还有很多韧带及肌肉的帮助来让它达到牢固的状态。    Sacroiliac Joint是由Iliac和Sacral結合在一起,當小孩慢慢成長之後,關節囊就會慢慢變為纖維化,缺少柔韌性及活動度。Sacroiliac Joint也由三個主要的韌帶及兩個次要的韌帶增加關節穩定度。

脊椎 Part 16 正确姿势

坐姿腰椎和骨盆角度不同,这将会严重影响别节脊椎。图中我们可以看到两种不同的坐姿,一个是理想的坐姿,另一个则是不好的坐姿。        当在驼背不好的姿势,这将会造成特定一些肌肉处于缩短的姿势,而肌肉缩短的情况将会造成腰椎疼痛。同时间这个姿势也是会拉扯脊椎后方一些韧带组织及弱化外层纤维环,容易形成椎间盘突出。        在驼背的姿势,上半身往前的力矩会增加,越大的往前力矩,椎间盘需要承受压力也越大。除此之外,头颈的位置也会受到影响,驼背的姿势,头和颈椎会往前倾,颈椎和腰椎侧边弯曲角度将会变小而影响正常该有的功能。 直立的坐姿是比较难维持的,这是因为腰椎后侧的肌肉在长时间久坐后会疲劳,疲劳之后就难维持在正常的姿势。坐姿是可以由自身醒觉的调整,强化一些肌肉群,增加一些肌肉群的柔软性,电脑的角度,人体工学的椅子和桌子等,都可以提供帮助。

脊椎 Part 15 腰椎和髋关节弯曲的相对关系

我们站着往前弯腰的时候,是需要腰椎(40度)和髋关节(70度)一起弯曲,如果某个地方有问题,另一个地方就必须代偿。如果Hamstring太紧,这会限制髋关节弯曲的角度,腰椎就不要弯曲更多。如果腰椎关节紧绷造成弯曲的角度受限,髋关节则需要代偿。所以任何一个部位有问题,相对也会造成另一个地方的负担,进而造成提早退化。 当我们从弯腰的姿势回到直立的姿势,肌肉的作用是有先后顺序的。首先是大腿后侧肌肉先出力,等到身体比较直立的时候,才轮到腰椎后侧肌肉用力让身体完全直立。当有某些肌肉力量不足,将会造成别的肌肉过度用力,而造成伤害。例如:当大腿后侧肌肉无力,腰椎后侧的肌肉就必须提早出力,这将会让腰椎L5/S1受到更大的斜力,最终容易造成脊椎滑脱。

脊椎 Part 14 椎间盘突出的分类

在健康的椎间盘是可以抵挡往后突出的压力,但是如果外层纤维环变弱,当中央髓核往后挤压,外层纤维环因支撑不住压力,进而增加髓鞘和神经线的压力。         一般上常见的椎间盘突出是后侧或后旁侧,这些方向是容易挤压到髓鞘或神经线。一般突出严重程度可以分为四种:Protrusion, Prolapse, Extrusion和Sequestration。如果是轻微的情况,一般症状表现只是腰部局部疼痛。如果严重往后,开始挤压髓鞘或神经线,症状就会延伸到下肢。肌肉无力或神经反射动作改变,都是神经组织受到压迫的现象。 两个机制是造成椎间盘突出的原因。 1. 是由一个很大,很突然的外力撞击,或扭转的力,例如:车祸,跌倒,拿重物等,中央髓核的压力迅速增加,而外层纤维环承受不住,就造成了突出。 2. 是由小幅度,但持续的力造成的,例如:长时间久坐,长时间蹲着,长时间姿势不良等,渐进式从小撕裂伤慢慢变严重。 弯曲和旋转的力是让椎间盘受损的主要原因之一。
